Your right to a Good Faith Estimate

Under the federal No Surprises Act, you have the right to know the cost of your care in writing before you receive it. Here’s what that means, and how to request one.

What the No Surprises Act guarantees you.

If you’re uninsured or paying out-of-pocket, federal law gives you specific protections around the cost of your care.

RIGHT 01

A written cost estimate, on request

If you’re uninsured or paying out-of-pocket, you can ask for a written estimate before scheduling any service. We’ll provide one within the required timeframe.

RIGHT 02

Estimate at least 1 business day in advance

For services scheduled at least 3 business days out, you’ll receive the Good Faith Estimate at least 1 business day before your appointment.

RIGHT 03

The right to dispute a bill

If your final bill is $400 or more above your Good Faith Estimate for any single provider, you have the right to dispute the bill through a federal patient-provider dispute resolution process.

RIGHT 04

No retaliation for asking

Asking for an estimate, requesting an itemized bill, or disputing a charge will never affect the care you receive at Horizon.

Good Faith Estimates, answered.

Under the federal No Surprises Act, healthcare providers must give an estimate of the bill for medical items and services to uninsured or self-pay clients before they receive care. The estimate must list each item or service and how much you’re expected to pay.

Anyone who is uninsured (no health insurance) or who chooses not to use their insurance for a service. If you’re using insurance, your insurer provides cost details through other means — your Explanation of Benefits and our pre-visit benefits verification.

If the difference is $400 or more for any single provider, you can dispute the charges through the federal Patient-Provider Dispute Resolution process at the U.S. Department of Health & Human Services. We’ll help you understand your options.
